All agreements signed between the clinic and Guardian are stored in your GuardianMD Portal. To access them, follow these steps:
1. Log in to your Clinic Account with your user name and password.
2. In the top right corner of the portal, click on your Clinic's name, and click "Profile" from the dropdown menu.
3. On your profile page, click the "Agreements" tab
Here you will find copies of all agreements signed between your clinic and Guardian. These can be downloaded for your own personal records by clicking "Action > Download".
