How do I upload and download documents from the portal?

Uploading and Downloading Documents Walkthrough


Here is step-by-step instructions on how to upload Documents: 



Step 1: Login into GuardianMD

To begin navigate to our Sign-In Portal by entering this URL in your web browser: https://portal.guardian-md.com/login


Step 2: Navigate to Documents menu

Click Documents from the left side menu.


Step 3: Upload Protocol Documents

From the Documents menu -> click Document -> then click Upload Document.


Step 4: Fill out Document details 

  1. Click the button to select Yes for Does this request need physician signature? 
    1. VERY IMPORTANT: You will need to click this button in order to obtain a physician signature successfully.
  2. Select the Physician Name from the drop-down list.
  3. Select a Document Type from the drop-down list. These are some options to choose from: 
    • Consent Form
    • Pharmacy Form
    • Vendor Form
    • Procedure
    • Standing Order
    • Insurance
    • Corporate Articles
    • Collab Agreement
    • MSO-MSA
  4. You will see the appropriate Physician Roles for the selected Physician Name.
  5. Upload the document by clicking or dragging in the Document section. 
  6. Click Save Document.


Step 5: SignNow

A SignNow Portal will pop up to place a digital signature upon the uploaded document for the assigned physician to sign. To do this:

  1. From the left hand side menu -> drag and drop the Signature field to the right place in your document.
  2. Click Save and Close.
  3. If successful, you will see a Congratulations window pop-up.
  4. The document will be in Pending Status in left side menu Documents


Step 6: Check Dashboard for Pending Document

Now that you have uploaded your document that requires a physician signature navigate to your personal GuardianMD Dashboard and check to see if there is a document pending a signature. 




So what happens next?

Now that you have uploaded your document successfully, the assigned physician will receive your document and review to sign the document. 


The physician can approve or reject the document. You will be notified with the outcome.


How to download the documents once physician has signed them?



Step 1: Navigate to Documents from Dashboard

  1. Click Documents from the left side menu.
  2. Click the Completed tab.
  3. Navigate to Action column, click on Action drop-down menu.
  4. Click Download Document -> The document will then be downloaded in your computer Downloads folder.
