In order to access your physician’s information such as their NPI number and their license details, follow the steps below.

Step 1: Log in to your GuardianMD account using your userID and password. 

Step 2: Click on “Physicians” in the left-hand side bar.

Step 3: On the list of physicians on the next page, find the one whose information you are searching for. Once you find their name, click on “Action” and from the dropdown menu click “View”.

Step 4: This will bring up a screen showing all your physician’s details. You can click on the “license” tab to view their license information and download a copy for your records.
