How to Upload a Chart Note

This guide provides step-by-step instructions for downloading chart notes from GuardianMD's Electronic Medical Record (EMR) system and uploading them in the GuardianMD Portal.

Note: If you haven’t created a chart note yet, please refer to the support document for step-by-step instructions.

Steps to Download a Chart Note

1. Go to "My Notes"

This is your workspace where all drafted & completed notes are stored.

2. Go to "All Notes"

This will show you a full list of your chart notes

3. Click on the "Completed" Tab

4. Filter by Date Range

Set the start date to the 1st of the month
Set the end date to the last date of the same month

5. Open the desired note

Click the eyeball icon to view the chart note

6. Download the Finalized Chart Note

Scroll to the bottom of the note and Expand the "Finalized PDF"
- Click "Download"

Steps to Upload the Chart Note

1. Navigate to the Guardian MD Portal (click here)

2. Navigate to the Chart Review Page

Expand the Documents section and click "Chart Review"

3. Request a Chart Review

4. Submitting your Chart Note

Select your physician and add a description
- Upload the chart note we downloaded from section 1 of this document

5. Save your Document

Once completing the Click the green "Save" button in the bottom right of the pop-up.

6. Verifying Chart Note Was Submitted

After successfully submitting the chart review, a green confirmation message will appear on your screen (as shown below). The most recent document listed in the table will be the chart note you just submitted
