Building Custom Intake Forms & Patient Encounter Notes
Your Guardian Medical Direction EMR allows you to build custom patient intake forms and encounter notes. This quick guide will provide instructions on how to build your own custom forms.
1. Login to your EMR account via - EMR Link
2. Select the "Settings" tab from the left-sided navigation menu.
3. Select the "Notes & Forms Templates" from the settings menu
4. From the "Notes & Forms Templates" page there are two separate tabs. Please select your desired option.
- "Encounter Notes Templates"
- These notes are filled out by your provider.
- "Patient Intake & Forms"
- These forms are filled out by your patient and can be sent via email.
Note - Your EMR comes with pre-made intake forms and encounter notes, these cannot be deleted. Although, they are useful to use as a template instead of building them from scratch.
You may copy a form by selecting the icon with two squares (shown below)
For this guide we will be creating a form from scratch by selecting the "Create New Form Template" button on the top right of the screen.
5. Enter your template name and description in the highlighted sections below.
6. Add your question types
For example - if you wanted to add the patients gender you can select "Checkbox" and enter out the required fields. If you need to add more selections. Utilize the "+" icon to add more (shown below)
7. To move a question around on the form, you must hover your mouse in the upper half of the desired question box. Your cursor will look like the screenshot below, click-hold and drag the question box to your desired location.
8. When you're finished with your custom intake form or encounter note, please select "Save" in the bottom right hand corner.
You will notice a green toast notification stating the record has been added successfully.
9. If you made a mistake, you can easily edit the form by selecting the "edit" icon in the "Notes & Forms Templates" page.
